Districts should be use their own content in addition to the state default

Some option to use state default content for the various
elements (checklist definitions, interventions, probe definitions,
etc)
This needs to carry over to the country default data if the state admin district has it set, and that needs to carry over to the system default data if the country admin has it set.

interventions checkists ...
include data from state admin
state admin should include country admin (if they have option set) country admin should include system admin ("")
Initially we can assume that the option to include is true, then we can store it somewhere.
