  1. Create a way for a staff member to generate a concern note.

  Add Team Consultation Form after Create Custom Flag

  When Team Consultation Form is clicked, a form [#269] opens

  After a user enters text and clicks on a save button, an automated email is generated to a predetermined recipient who receives all referrals for team discussion and creates the team schedule
  E-mail text:
  

  A team consultation form has been generated for (name of student) on (date) by (person entering consultation form).  Please schedule an initial discussion at an upcoming team meeting.
  

  When an email has been successfully sent, the screen includes the following message:
  

  The team consultation form has been sent to (name of person).  A discussion about this student will
  Occur at an upcoming team meeting.
