When multiple uploads present they're lost on validation failure

This works in production but not in dev, I'm assuning it's due to an upgrade and naming.
Fix explicit names on multiple file fields. If a file field tag has the multiple option, it is turned into an array field (appending []), but if an explicit name is passed to file_field the [] is not appended. Fixes #9830.
This is currently blocking an upgrade to 3.2.16 (from 3.2.13) It was also not caught by tests.
To recreate:
Go to a student
add a team note
Add 2 attachments
Leave the title blank
Hit save.
The attachments should still be there but only the first one is there now (and it appears to be saved.)

It fails in production too, this is not related to the ugprade.

When #720 is complete, we will need to switch to a different test case as team notes with attachments but no title will be valid.
