Alternate formats for team consultation forms?

An idea to address this.
Basically:
1. The school admin attaches blank forms to the teams.
2. When someone completes a team consultation request or form, the blank documents are available there,and then they have the option to download it, fill it out, and upload the completed version.That way users could use the existing team consultation form, or upload their own attachments.

hide the form if it has no content and the selected team has assets
toggle appropriately on change of the recipient
